HOPE Atlanta’s Projects for Assistance in Transition from Homelessness (PATH) Outreach Program assists unsheltered persons by engaging them where they are located, ensuring they access any treatment they need (mental illness, physical health, substance abuse), and then placing them in housing coupled with case management.

While any unsheltered person can benefit from these services, generally speaking the program targets the chronically homeless, those who are suffering from severe mental illness, and especially those who have been homeless for longer than one year.

Approximately 30% of the people receiving services from this program fit HUD’s definition of chronically homeless and over 55% have been diagnosed with severe mental illnesses.

The primary purpose of the PATH Outreach Program is to engage clients living on the streets and places not meant for human habitation, stabilize their mental health, physical, and substance abuse issues by accessing medication and treatment programs, and ultimately connect them with housing. Cooperation with the Outreach Team is strictly voluntary on the part of consumers, therefore any given individual may be engaged numerous times before finally agreeing to accept services. After the initial intake and assessment has been completed, a variety of services are provided depending on each individual’s unique needs. The PATH Team operates primarily in the City of Atlanta and Fulton County, though it expanded its services to DeKalb County in 2011 through a partnership with the DeKalb Community Service Board (CSB). 218 unduplicated persons were served by this program in FY 2012.
